We’re going to be talking about some of those over the several weeks. But before we dive into anything else, let’s revisit one topic that stays urgent all year round.
You might think that these extra hours of sunshine mean that you can ditch your vitamin D supplement. But here’s your friendly reminder to keep taking that daily dose regardless of daylight savings…
A year-round challenge
Fact: Most people simply don’t live close enough to the equator—or get outside enough—to achieve adequate vitamin D levels on sunshine alone at any time of year. And rates of vitamin D deficiency among Americans are sky-high because of it.
Unfortunately, the science on this subject is very clear: Vitamin D deficiency leads directly to higher risk of disease—and not least of all, to a higher risk of prostate cancer.

Vitamin D by the numbers
First: Everyone should get routine vitamin D testing. The absolute best way to determine adequate levels of vitamin D is with a 25-hydroxy-vitamin D blood test.
Some practitioners order it automatically. If your doctor doesn’t, ask for it. And when the results come in, make sure you ask for the numbers, too.
That’s because many doctors consider anything over 30 ng/mL to be sufficient. This is the deficiency threshold for most clinical research—but for optimal health, you want to your levels to be much higher.
Dr. Geo likes his patients to be between 40 and 70 ng/mL. Anything below that requires aggressive supplementation. And what does that mean?

Resistance is key
This might seem like a no-brainer. After all, as we explained last week, obesity is a leading cause of low-testosterone, and getting regular exercise is one of the best ways to keep your weight in check.
But research suggests that its effects on your testosterone levels are independent from weight. In fact, studies show that physical activity delivers a significantly greater testosterone boost than calorie restriction alone in overweight men.
Of course, research also shows that not all forms of exercise are created equal where testosterone is concerned. And some approaches are more effective than others.
Not surprisingly, resistance training leads the pack here. Studies indicate that weight lifting is the best way to boost testosterone in both the long and short term: Training for three days a week has been shown to raise levels significantly within just four weeks.
But high intensity interval training has also shown testosterone-boosting benefits in men. (This type of workout combines short, intense bursts of activity with less intense recovery periods—a 90-second sprint, followed by 90-seconds of walking, for example.)
At the end of the day, though, any exercise is better than none at all. With one caveat…
Don’t overdo it
A combination of cardio and strength training—adding up to a few hours of moderate intensity exercise per week, with plenty of movement throughout the day—is key. But you might want to think twice about extreme endurance exercise, such as marathon training.
This type of exercise can have the opposite effect—sending stress hormones like cortisol soaring, and crushing your testosterone levels in the process.
Remember, the goal here is to reduce stress, not increase it. Work hard—but not too hard. And make sure you’re getting the rest you need to recover in between.

So today, let’s take a closer look at this critical sex hormone—where it comes from, what it does, and why imbalances can wreak havoc both inside and outside the bedroom...
Keeping the balance is key
The testes produce the majority of your body’s testosterone, using cholesterol as a building block, while a small remainder originates in the adrenal glands. The process begins in your hypothalamus and pituitary gland—a part of the brain known as the HPA axis.
When testosterone is low, your hypothalamus signals your pituitary gland to release luteinizing hormone. This hormone tells cells in your testes (called Leydig cells) to start producing more testosterone.
Testosterone is then carried into your bloodstream by a protein called sex hormone binding globulin (SHBG), where it’s responsible for a whole host of critical functions—supporting muscle mass, bone density, sexual function, and more.
But of course, it can only do this if enough testosterone is unbound to SHBG—what we call “free testosterone”—allowing it to easily interact with your cells and tissues.
There is a balanced feedback loop in play here: When your body has adequate testosterone levels, your HPA axis slows down the production of testosterone-producing hormones. The same thing happens when you have high levels of other sex hormones, including Dihydrotestosterone (DHT) and estrogen.
Yes, men need estrogen, too: It’s critical for libido and sexual desire—but an imbalance, and high levels in particular, will have a negative impact on your sexual performance.

Can soy, a popular meat substitute, protect against prostate cancer?
Soybeans contain fiber, polyunsaturated fats, potassium, magnesium, and other vital vitamins. It also has all the required amino acids—the building blocks of protein—which is why soy is popular among vegetarians and vegans as a source of protein.
Soy products are broken down into two categories:
One major research study shows that two ingredients in soy, genistein, and daidzein, are associated with a lower risk of prostate cancer. Further, unfermented soy food products were associated with a decreased risk of prostate cancer, while fermented soy food products had no associations with prostate cancer risk.
The rates of prostate malignancy are lower in Asian countries (where soy is a diet staple) compared with Western countries. This could be due to equol, a by-product of soy gut bacteria produce. Some research has found that equol is associated with a lower prostate cancer risk. However, not everyone can break down soy to make equol. One study found that American men produce less equal than Japanese and Korean men. So, can non-Asian men make enough equol from soy to protect themselves against prostate cancer? Likely not.
Even though isoflavones in soy are a type of phytoestrogen, which may have a similar effect on the body as estrogen, one research article has shown that soy does not increase estrogen levels in men.
There is much confusion about soy and prostate cancer. Whether it may protect against prostate cancer or help manage your diagnosis is unclear. Until then, I think it's perfect for men to enjoy soy in moderation. Also, it's not a good idea to think of any one food as a panacea. At XY Wellness, we believe the whole integrative roadmap is better than the sum of its parts. Eating a variety of optimal, clean foods that include broccoli, brussels sprouts, cauliflower, and cruciferous vegetables is best – they are called and have been shown to have prostate cancer protection.
Final Thoughts and Takeaway from Eating Soy
Later in our dinner last night, I passed on the soy ice cream. I'd avoid fake soy products like soy ice cream, soy burgers or soy hot dogs. Fake foods are crap too. If you want ice cream, have ice cream. Feel free to consume one to two daily servings of soy in the form of tofu or tempeh. A couple of cups of soymilk a week may also be beneficial and contain benefits in fighting prostate cancer. Again, one food is never a cure for any disease. Cancer is a complex and multifaceted group of diseases, and no magic bullet can eliminate it entirely. However, a balanced, whole foods diet can significantly reduce the risk of developing and progressing prostate cancer.

When we are diagnosed with prostate cancer, the immediate psychological effect is a sense of loss of both health and control. In order for us to provide you with our premium products at the best available price point, we avoid the material price increase that is necessary to “feed” this and other reseller channels. I don't treat prostate cancer. I treat my patient's biological environment with nutritional and lifestyle approaches. You see, treating prostate cancer with surgery, radiation, or focal ablation is only one part of the puzzle. The other critical element is treating the terrain, the host, you.
Recently a patient sent me an article by Dr. Siddartha Mukerherjee, MD. His article, Cancer's Invasion Equation, focuses on treating the biological terrain. In this article, Dr. Mukerherjee writes "A tumor is made of a heterogeneous mixture of millions of cells, only a fraction of which are equipped to leave the primary tumor, form an exploitative alliance with the 'soil' of another organ, and initiate metastasis."
In the same article, Dr. Joan Massagué, a cancer biologist at New York's Memorial Sloan Kettering Cancer Center, explains "When tumor cells enter the circulation, they must perish almost immediately, and in vast numbers. Only a few reach their destination organs, such as the brain or the bone. Once they do, they face the additional problem of surviving in unfamiliar and possibly hostile terrain. A visible, clinical metastasis—the kind that we can detect with cat scans or MRIs—must only occur once a dormant cell has been reactivated and begins to divide. Malignancy wasn't simply about cells spreading; it was also about staying—and flourishing—once they had done so."

I am virtually certain your supplements have a real benefit. Initially, my PSA dropped from 3.4 to 1.8 over the first year, and then stayed in the low 2’s for years. After many years on your program, I somehow “fell off the wagon” in part because I could not tell if I was benefiting and in part because of the expense of the supplements. My PSA rose to 3.4 in the Fall and then to 4.4 in the Spring. At that point, I was alarmed, and made an appointment with Dr. Geo to review my health plan. The PSA score may not represent the be-all of prostate cancer disease tools, but it’s what we have. Get a blood draw at least annually. Skip it and you may be in peril.
Second, don’t listen to anyone who says he’s “cancer free” because that’s just denial. I had a prostatectomy in 2003 and a recurrence (after watching my PSA slowly rise) in 2021. After 8 weeks of pelvic radiation and 6 months of ADT, my PSA fortunately remains at 0.008.

No blood test provokes more anxiety in men than the Prostate Specific Antigen (PSA) test. Not cholesterol, not glucose, not triglycerides—nothing else makes a man's knees shake like when he goes for a PSA test.
You know what PSA stands for, right? Patient Stimulated Anxiety. Kidding aside, the problem is that feelings of worry and stress can cause the PSA number to rise by itself. So, it would be in your best interest to figure out some tools and techniques to remain calm about PSA.
A high PSA does not always mean cancer and certainly does not always mean death will occur after prostate cancer.
To further confuse things about this anxiety-inducing biomarker, David, a prostate cancer thriver for 19+ years and my partner at XY Wellness, LLC, wrote a great blog post about Dr. Richard Ablin, the creator of the PSA test, titled "PSA Test: How to Properly Interpret and Avoid Misuse" Dr. Ablin has argued that PSA can't differentiate between the type of prostate cancer "that kills you and the one that won't.… The test simply reveals how much of the prostate antigen a man has in his blood…. Men with low readings might still harbor dangerous cancers, while those with high readings might be completely healthy."

How Testosterone Works
Testosterone is a hormone produced primarily in the testicles of males (and the ovaries of females.) Testosterone has many bodily functions, including regulating bone density, red blood cell production, and fat distribution. It also has effects on muscle building, mood, and cognitive function. This hormone declines naturally with age or has therapeutic value in men with a certain level of prostate cancer which can lead to a range of symptoms, including decreased muscle mass and strength, cognitive decline, bone loss, and hot flashes (night sweats).
How Androgen Deprivation Therapy works for Prostate Cancer
Androgen deprivation therapy (ADT), also known as androgen suppression therapy, is a treatment for prostate cancer that aims to reduce the levels of male hormones called androgens in the body. Androgens, such as testosterone, can stimulate the growth of prostate cancer cells. By lowering the number of androgens in the body, ADT can slow down or even shrink prostate cancer tumors. ADT can be achieved through various methods, such as medications that block the production of androgens or surgical removal of the testicles, the primary source of testosterone in the male body. ADT may be used as the primary treatment for prostate cancer or combined with other treatments, such as radiation therapy.

What are free radicals?
They are volatile molecules that interact quickly and aggressively with other molecules in our bodies to create abnormal cells. Free radicals are not always a problem as they are made from the normal metabolism of food and are a byproduct of the detoxification process. Although free radicals have valuable bodily functions under controlled conditions, they are extremely unstable molecules that can damage cells if left uncontrolled. Also, the production of free radicals increases with age – and many diseases, including prostate cancer, come with aging. Free radicals damage cells in various ways, penetrating the DNA and damaging its "blueprint" so the cell will reproduce uncontrollably. The unfortunate result of too many free radicals in our body is accelerated aging and the development of many diseases, including cancer and heart disease.
What are Antioxidants, and how do they work?
An antioxidant is a molecule that helps protect cells from the damaging effects of free radicals. Antioxidants scavenge free radicals and stabilize the unstable free radicals by giving them the electrons they need to "behave" and act as normal cells. An ideal antioxidant should be readily absorbed by the body and should prevent or quench free radical formation or cling and dispose of metals to keep a prooxidant/antioxidant balance. Unfortunately, antioxidants are usually consumed or used up in this process—they sacrifice themselves to keep a cell healthy. That's why a constant amount of antioxidants consumed from food and nutraceutical supplements is essential. The most notable antioxidants that offer protection are vitamins A, E, and C, glutathione, bioflavonoids, selenium, zinc, and various phytochemicals from herbs and foods. Green tea, for example, is rich in polyphenols—powerful antioxidants that help fight cancer.
